The opportunity
This is a meaningful opportunity to join a values‑driven community health organisation making a tangible difference in people’s lives. You will work directly with community members who are often under‑represented or hardly reached by services. The role focuses on outreach, engagement, supported referrals and building strong community connections. You will collaborate closely with internal teams and external partners across health, social and community services. This position offers variety and the opportunity to contribute to service improvement and innovation.
About the role
We are seeking a Community Outreach and Engagement Worker to join our Community Impact team in a permanent part‑time role (0.5–0.6 FTE) Tuesday, Wednesday & Thursday based in Hawthorn or Doncaster. Reporting to the Manager Community Access and Outreach, you will work in a team with community health nurse and outreach worker to facilitate connections to community members who need linkage with health and social supports within local community areas of Boroondara, and Manningham.
